Claims Issue:

   * StudentResources has a different claims address than United Healthcare, in fact, the are a completely different division.  Many providers were incorrectly filling claims with the wrong insurance company.  This is difficult to spot since the rejection will come from United Healthcare and look like it is from our insurer.  See:

and look for, "How-to: Correct the in-network claims errors from your providers."
   * For those of you who have had claims rejected or know people who have, please ask them to first contact their provider and have them refile the claim under the correct electronic filing code that is on the back of your ID Card.  If you have any questions, please contact customer service.

Customer Service

   * There have been many complaints about Customer Service.  In order for us to address those complaints we need to provide the calls that we want to use as examples.  Please fill out the survey at: http://gpsc.duke.edu/survey/StudentResources_Customer to tell the GPSC Committee about your experience.  You do not need to provide your any self-identifying information (except to log-in) if you don't want to, but we will need your name and phone number that you called from or SR ID number to track the call.

To enroll or waive out of the insurance:

   * Duke has an insurance requirement.  If you have comparable insurance, you do not need to have this plan.  This must be done before the end of the open-enrollment window at the beginning of each academic year.  The electronic forms are located at:

   * For other enrollment questions, such as enrolling an dependent (mid-year), contact customer service.

Beaufort Providers:

   * Students at the Duke University Marine Lab in Beaufort, NC should be aware of the network increases that have been made but not on the website yet.  A listing of these will be posted on the GPSC Insurance page.
   * If you know of a provider that you would like to have added to the Duke network, please send the name, address, and phone number to the GPSC Health Insurance Committee.

Student Medical Insurance Plan HOW TO Guides

In this section we will list how to navigate our new health insurance policy.

HOW-TO: Create an account to check your claims, print a temporary ID card, and many other tasks

Go to www.uhcsr.com/duke.  On that page there is a link to 'Create an Account' on the left navigation bar - click it and follow these steps:

  1. Enter your first and last name as well as your date of birth.
  2. Enter your e-mail address; or, if you have your ID card, you may enter your SR ID number.
  3. Choose an username and password.
  4. You should now be logged in.  Remember to log out when you are done.

HOW-TO: Correct the in-network claims errors from your providers

Many providers are incorrectly routing claims to the wrong claims administrator.  Attached is a letter (at the bottom of this page: USCSR_Claim_Submission_info.pdf) that explains how to submit the claim to the correct claims administrator.  If you experience problems from your provider filling an in-network claim, give them this letter; in most cases it will solve the problem.  This happens because we have a United Healthcare StudentResources policy - not a United Healthcare policy.  The StudentResources division is owned by the parent company, and as you have experienced, we all use the same provider network; however, they use completely different and isolated claims administrators.  This is actually good for the providers - the bad press concerning late payment of claims is specific to United Healthcare.  StudentResources claims administrator is actually prompt with payment.  Once providers understand this difference they can route your claim correctly.  Furthermore, providers that are not in-network are more willing to deal with the StudentResources policies because of their superb record with claims payment.

HOW-TO: Add a new dependent

According to state law (most states are similar), you can only add a new dependent to your (the student) policy during open enrollment (approximately Aug. 1st to Sept. 15th) or within 30 days of a qualifying event.  A qualifying event includes, but is not limited to marriage, birth or adoption or an eligible child, or loss health insurance as a result of loss of dependent status.  To add a dependent, you may call Customer Service at 1-800-548-8472 or fill out the dependent enrollment form attached below (2008_DependentEnrollment_v1.pdf).  The rates for adding or removing a dependent for a qualifying event should be dated for the full month that includes your enrollment date.  The forms do not reflect this yet and your best bet will be to call customer service.  If you have any problems, contact dukestudentinsurance@gmail.com.

HOW-TO: Submit a claim for your eyeglasses or contact lenses

United Healthcare StudentResources would like you to simply send in your receipt from your purchase for up to $100 reimbursement from them.  For your convenience, GPSC has a form letter for you to fill out so that you remember all of the information.  Please fill in this MS Word form - please fill out the form (all the gray fields) completely with the exception of the date, that will update automatically.  The letter is attached to this page at the bottom: Eyeglasses_reimbursement_letter.doc.

If you wish to submit a claim for another benefit, such as out-of-network provider or durable medical equipment, you can use the included letter as a template.  The personal information is the same.  Just change the context to explain the charge/purchase and cite the benefit listed in the benefit book (select 2008 policy brochure).

HOW-TO: Use our mail-order prescription benefit and take advantage of a three month supply for the price of two

The first time you use the mail-order service for a drug you will need to send in two forms and the original, signed, prescription form.  It is recommended that if you need to start the drug you obtain the first month at a pharmacy.  Attached below is a file called MedcoForm.pdf that contains the instructions and both forms that you will need to mail or fax to Medco.  Please see the attachment for detailed instructions.

HOW-TO: See a provider in Beaufort, even if there aren't any listed

In case of a true emergency you should not hesitate to seek emergency care - there is no in-network versus out-of-network for emergency departments.  Incidentally, the local hospital, Carteret General Hospital, is in-network. Our contract with UHCSR asserts that if there is no provider within an area that is in-network for your needs, you may see an out-of-network provider for in-network rates.  If you are unsure if this applies to you, contact United Customer Service with your specific needs.  DUML Administration has more information on this.  You may have to pay for services up front, but you will be reimbursed as long as there is no in-network provider in the area.  The claims procedure is outlined in the benefits book.

HOW-TO: Access one of the dental discount plans available to all health insurance participants

We have several discounts available to you - they are part of your health insurance, so you do not need to do anything else to access the discounts.  We suggest printing off the listing from the dentist that you choose to help them process the discount.  You may also be asked for your Duke ID and/or insurance card (gray card) or UHC Health Allies card (white card) that you received at the beginning of the year.  If you cannot locate your UHC cards you can print a new one by going to www.uhcsr.com/duke and going to MyAccount.

UnitedHealth Allies: http://www.sr.unitedhealthallies.com/
Register on the right using the Health Allies card number (white card) that you received in the beginning of the year.  Then search for a participating dentist under "Find a Provider" near the top of the page.  Select Dental, General Dentistry, then the zip code (Duke University is 27708).  This will give you a listing of dentists that give a discount through United.

If you haven't done so already, follow the above link for instructions
and forms for completing your health history and immunization record
(which can be done online).  THIS IS REQUIRED BY THE STATE OF NORTH
CAROLINA.  You will need the dates of your immunizations which can be
found on most physical examination forms; alternatively you can get
this information from your previous primary care physician or
undergraduate Health Center.

If this is not completed BY TUESDAY, SEPTEMBER 30 you will be
DISMISSED from Duke University.

(A new student is a student that matriculated in the Summer or Fall
2008 terms within any branch of the University.  Many health
profession programs have additional requirements that you may have
filled - please check with your department or Student Health at
DSHS_Immunizations@mc.duke.edu if you have questions.  This is a
one-time requirement so returning students do not need to submit any

United Healthcare StudentResources Questions

Your Duke Student Medical Insurance Plan

There are many changes to how the new plan operates. As we start the school, and policy, year, the following information will be helpful in getting you squared away with your new insurance plan. The topics are written in chronological order, so read over the subjects (bold) of each item to make sure you haven't missed a step.

Introduction
The new plan is administered by United Healthcare StudentResources. This is a special division of United Healthcare that deals exclusively with student plans. They perform most of their services independently from United Healthcare, but still carry the widely recognized name and the associated nationwide network. The new plan maintains almost all of the benefits of the previous (Blue Cross and Blue Shield) plan, so if there is a benefit that you are confused about, let us know and we can help you out. Am I enrolled?
If you are a full-time Duke student and are subject to the Student Health fee, or your department typically charges the Student Health fee, and you have not waived out of the plan, you are enrolled and your coverage begins on August 16, 2008. There are some departments that are not eligible for the Duke Medical Insurance Plan (SMIP) and include some Masters programs and Professional programs. The vast majority of students on campus are eligible for this plan including undergraduates.

If:

  • You would like your ID card to arrive at the beginning of your coverage or as soon as possible or
  • You need to enroll dependents or
  • You would like your coverage to begin on August 1, 2008 and you are not a new student,
    or
    If you don't want this plan

You will need to "positive enroll" by visiting https://www.srstudentcenter.com/Default.aspx?schoolId=243. Near the bottom there is a link for "Enrollment form for students" - fill this out to enroll. You must fill this form out before enrolling your dependents. This link will bring you to a page that requests your date of birth and student ID or social security number. Enter your date of birth and your Duke Unique ID (do not bother with your social security number as Duke did not share this information with United). If you don't want this plan, you may apply for a waiver if you have comparable insurance by clicking on the waiver card (you will still need to sign is as described here).

You can find your Duke Unique ID number on your University Identification Card on the back (see:
http://dukecard.duke.edu/card_numbers.html for a picture) or new students can find their Unique ID number by logging onto ACES:
http://www.siss.duke.edu/siss/aces (on the new ACES) your Unique ID appears on the top of the first page.

To enroll dependents:
If you would like to enroll dependents on your plan (spouse, same-sex-spousal equivalent, and/or dependent children) follow the same link as the last step and (after you have positive enrolled yourself) select "Enrollment form for dependents." Fill in the information of the dependents that you want covered on your policy and select the terms that you want to pay for at that time. You do not have to pay for the whole year at once. To pay by semester, only select the fall. In December you will receive an e-mail reminder to go in and pay for the next term.

I'm/We're enrolled, where is my/are our ID cards?
Your insurance cards will be mailed to your address on file with the University after you have filled out the positive enroll form, or after September 15, whichever is sooner. If you would like your ID card sooner than September 15th, please make sure you have filled out the enrollment form. If you would like to obtain a card from the website, follow these steps:

  • Go to https://www.uhcsr.com/duke
  • On the bottom of the page, click on "My Account"
  • If you have not already created an account, click on "create My Account now" in the text under "My Account Login"
  • You will then create an account using your name and date of birth

Under the "My Account" menu, there is a section called "ID card information" and below that is "view or print ID card." Click on that to view a custom .pdf file with your card information.

Student Health, your preferred first stop
It is typically more cost-effective and timely to visit Student Health for all of your medical needs or Counseling and Psychological Services (CAPS) for your mental health needs. Most situations can be taken care of there, but there are times when you will need outside help for yourself or dependents (dependents are not seen at Student Health).

The reason that we advocate using Student Health and Counseling and Psychological Services (CAPS) is that the cost is much lower than outside providers. These small, Duke student-only clinics are run so efficiently because they are serving a young, mostly healthy population. Furthermore, students pay for these services with the Student Health fee, so they are already paid for, you might as well use them. The Duke Student Health pharmacy can also provide some medications at a lower cost than outside pharmacies due to bulk purchasing through the hospital (Although current federal law has greatly limited this, there are some items that are still cheaper at Student Health - lawmakers are working to change that restriction).

In-network providers
Even though you should go to Student Health for practical reasons, you don't have to - ever. You don't need a referral. So, should you have to venture outside of Student Health and CAPS, it is always a good idea to use an in-network provider. These providers (clinics and hospitals also) have a special arrangement with the insurance company to provide services at a reduced rate. In addition, services from in-network providers are paid at a higher percentage than services from providers whom are not in-network. Consider this example of two identical doctors providing a service for the same price, $100; except one is in-network, the other is not.

  • In-network
    • Base price: $100
    • Discount: $40 (this is the amount that is negotiated for by the insurance company - you get it just for belonging to the plan)
    • Amount insurance company pays: $48 (that is the remaining $60 x 80% = $48)
    • Amount you owe: $12
  • Not in-network
    • Base price: $100
    • Amount insurance company pays: $70 (that is $100 x 70%, since providers that are not in-network are only paid at 70%, not 80%)
    • Amount you owe: $30
  • And, just for fun (although it's not funny), if you don't have insurance
    • Base price $100
    • Amount you owe: $100

As you can see, it is advantageous for the customer (you), the plan (Duke students who pay for the amount the insurance company pays), and the insurance company to stay in-network whenever possible. Of course, you can also see why having insurance in the first place is important - most bills are not for $100, they are more like hundreds or thousands for more involved services.

Ok, how do I find an in-network provider?
If you have to visit a provider outside of Student Health or CAPS, try to get a referral from Student Health or CAPS - remind them, but they should be able to locate an in-network provider (as long as there is one for the specialty you seek). They can also usually get you an appointment sooner than if you go on your own. But should you have to, for yourself, or your dependents, you can search the list of in-network providers from https://www.uhcsr.com/duke: click on "UHC Choice Plus" for in-network providers, or "United Behavioral Health" for mental health providers.

You may still need help selecting from all of the providers. It is a good idea to use the GPSC Forums at http://gpsc.duke.edu/forums to get recommendations for providers.

As a part of our contract, all Duke Medical Center providers, including the hospital are in-network; however this does not necessarily include the Physicians Diagnostic Clinic, which uses the same logo. You can always ask a provider if they are in-network. There is a caveat in our plan that a provider will be treated as in-network is there is no suitable in-network provider within a certain distance. You will have to contact Customer Service for help with that.

If you are not in-network, you may have to pay the full amount for services and be reimbursed from the insurance company. You can find instructions on how to do this under My Account on the United Healthcare Student Resources website - instructions for creating your account are in the ID Card section above.

Dental Plan
We have two options for a dental plan. There is a discount plan that is included with the insurance plan at no extra cost and without signing up. Details can be found at http://www.basixstudent.com/duke/. If you require dental insurance, United Healthcare is offering a plan $357.84 per year that covers a wide range of services. Details can be found under "2008 Dental Benefit Summary" on https://www.uhcsr.com/duke. The form is also available to sign up.


Preliminary Dental Plan Information

The 2008-2009 health insurance policy will INCLUDE a dental discount plan. An optional dental insurance plan will be available at an additional COST. Information on both plans are available in the attachment.

Health Insurance Recommendation

Please see the attached .pdf file of the presentation made to the general assembly on March 26.

The recommendation is for Duke to contract with United Healthcare/StudentResources. The recommendation also comes with serveral plan changes such as a copay schedule. There will be an included dental discount plan and an optional dental insurance.
